Form not showing up in checklist

I have a classic onboarding checklist and adding another Action Item. The Item is a Form but when user gets to the Item it just shows the Submit button and nothing in Instructions tab. It should show Employee id and Electronic signed date so nothing unusual. There are other Forms in the 26 step checklist and just mirrored those.

How does the Form get associated to the Item in the Activity Guide?

The Form is in the Acknowledgement menu and works as expected.
