OAB Not Calculating Rate Correctly

We have setup some new programs and are running into an issue where (even though our setups appear to match the options/plans/programs/rates used for similar ones), when we go to enroll an employee in the program, the rate is not correct.

For example: Monthly Rate $10, should be $5pp (biweekly pay, benefits pull from 24checks).

However, OAB enrollment screen says $5.22pp instead.

It's like it isn't picking up/considering the 1st check it's active on in it's 24pp calculation --- where do we go to fix this?
