Where can I find details on the parameters for the Audit Report (PDF)?

Oracle EBS 12.2.9 - on prem

I am having some difficulty finding the specifics for using the Audit Report (PDF) with responsibility: US Super HRMS Manager. I can access the report in concurrent manager but am at a loss for how to select the parameters.

I have reviewed the Oracle documentation and could be missing the information.
