OffBoarding not assigning Activity to Manager and Employee correctly

We are planning to implement Offboarding in our organization but when we do a termination from Job Data the Activity Guides does not get assigned to the Manager or Employee correctly. It stays with the super user who did the job data transaction.

We tried using the terminate employee tile as well from MSS, upon all approvals we dont see the Activity Guide created for Manager and Employee. When we go in as HR admin, the Trigger is created and when we click SAVE again it assigns the activity guide to the Super User and not to the employee and manager.
