Fluid Benefits - Enrollment Links

We are implementing fluid benefits. We are setting up the "Contacts" and "Resources" links on the right panel of the enrollment page. We have different providers for plan type 1X. How can we set-up contact information for Plan Type 10, 11, and 14? We tried setting up the links on the Provider table but only the link for Plan Type 10 displays.

We also have two providers employees can select from for dental (plan type 11) and vision (plan type 14). Is it possible to add two providers contact information and their links to the right hand navigation or would we have to customize?

Any information for setting up Contacts and Resources would be helpful.
