FRM-92101 issue

Dear All,

During upgrading Oracle EBS 12.1.3 to EBS 12.2.11, and while checking Concurrent Programs and reports (View==>Requests==>Submit a New Request), most requests ran as expected, but some did not. After selecting a request, and expecting parameter window to get opened to set parameters; FRM-92101: There was a failure in the Forms Server during startup. This could happen due to invalid configuration issue raised.

For Example:

Order Management==>Diagnostics: Apps Check

And

Order Management==>Interface Trip Stop - SRS

What we did; we created a new custom request based on Diagnostics: Apps Check executable (OMCHECK), then we added it to a request group. This custom request ran as expected.
