Multiple POs getting created for the same requistion

We are facing an issue where there are multiple POs getting created for the same requistion. Unfortunately, we could not replicate the issue in non-prod. Referred to the same issue in KM, however the scenario for those is not matching ours. Per KM the issue happens when there is an action on PO(cancel). In our case, the PO remains untouched. Please share any ideas/insights which would be helpful is issue replication/resolution. The issue requisition has since been canceled by business.
