Does patch apply in one node succeeds while fails in the other node in RAC environment?

I am trying to apply the latest GI PSU patch for 11.2.0.4.0 in Solaris 11. I have successfully applied the patch in the first node, but when I apply the patch using the same method in the second node, it somehow fails. Can someone provide any information regarding this issue?

Product -> Oracle Database Enterprise Edition (RAC)

Current version --> 11.2.0.4.0 on Solaris 11
