One-off patches and patch releases

in Install, Upgrade, Clone, Patch - EBS (MOSC)

Hello Oracle Family ..

We recently had to apply a couple of "bug fix"/"one-off" patches to address a crash of our EBS PROD system. Is there any way these patches could have been applied prior to the crash or are one-offs like these applied only when an issue is encountered?

Are one-offs like this addressed in future releases of the product, ie a Release Update or "minor release"?
