Duplicate Record In fnd_log_transaction_context

in Technology Stack - EBS (MOSC) Question

We have a created custom Integration Repository plsql interface . This is to use the REST API Service provided by Oracle

This works well but ocassionaly a duplicate record gets inserted into fnd_log_transaction_context and the service stops working. I could find the issue of duplicate record by checking the logs , deleting the record will make the service to work properly again. There is no fixed pattern on when this issue occurs, so difficult to reproduce

Please let me know if this is a known issue and if any fix exists to fix the problem
