Revised PO approved automatically

Hi,

 We have done some dummy revision and changed the tax for already approved PO. Then PO status changed as pending reapproval. Once we forwarded the PO to their HOD, but PO approved automatically. PO hod only check the taxes and approve the PO. Please help us in this regard.
