Receiving error Routine &ROUTINE received a return code of failure

hello all, Getting this error: Concurrent Manager encountered an error while attempting to start your immediate concurrent program PROJECT_TASK_EXTR. Routine &ROUTINE received a return code of failure. Contact your support representative.

This is a SQL package I am trying to run from concurrent program. No errors found compiling pkg, runs fine in SQL developer.

Not sure where the problem resides. Nothing is printing in the log and do not have access to request log. Any help is appreciated.
