Value Allocation Rule Not Found. Unable to Allocate

I am seeing these more often and I cannot find what is missing on a Buy or Sell shipment when one of these is returned after attempting to allocate. We have the allocation rule setup, and it works about 99% of the time, but these are occurring more often, and across all accounts.
