Periodically an orphan record get inserted with Blank DOCO in F4301,F4311,F4201& F4211,any idea why?

Periodically an orphan record gets inserted with Blank DOCO in F4301,F4311,F4201 & F4211 which prevent users from creating orders and displaying error "Record invalid" while the user create a Direct Ship order (SD/OD) The orphan record is deleted from the SO and PO tables as a work around .

Have you seen this kind of issue and is this a system glitch or any way to prevent it from happening since it is a show stopper
