Alert that requires a response

Hi Guru's

I have setup a new alert that requires a response. My alert goes out via email successfully, however my response email is not getting processed.

How can I track what is wrong? Are there any proper documentation that explains with example screenshots of how to setup an alert that requires a response?

Any help would be appreciated.
