IP Update didn't update some DLLs and EXEs

Has anyone had issues installing IP patch that didn't update some DLLs and EXEs? Example: siebmtshmw.exe still showed as previous version when checking Details. We installed on six servers but only one had this issue. Restored and repeated install and same issue. I verified these files were not in use via Task Manager before repeating the installation.
