Fluid Apply for Graduation

We are preparing to implement the Fluid Application for Graduation functionality. I have walked through the Supplemental Documentation for Bug 33841632 - SR FLUID: APPLY FOR GRADUATION (Doc ID 2861238.1). 

I am now getting a 'You are not authorized for this page' error when trying to access the activity guide as a student.

What properties/permissions need to be set on the delivered Application for Graduation activity guide template? Or is there another cause of this error?
