How do you add an image to hover form in E1 9.2.5.3?

I've read Oracle Support document FDA: How to Enable and Use Hover Viewer Forms in Application Releases 9.1 and 9.2 (Doc ID 1451431.1), but it is still unclear to me.

I'm using P4210 and adding an attachment to the line item detail. I still cannot see my image when I hover over the item number.
