ACA Box 15 Blank on Print Forms

Hi there! We're working on generating the IRS files and print file and noticed that no one's showing the lowest cost on box 15. However, it does show up on the ACA Employee Transmittal page.

Any thoughts about why it's not showing on the print forms? The cost is on the medical plans, I've verified the attribute pages are correct.
