Receiving an AIRSH100 rejection and can't figure out what is wrong

Here is the message:

XML Schema Validation Failed - not well formed or missing required elements

Form109495CTransmittalUpstream/Form1094CUpstreamDetail/Form1095CUpstreamDetail/CoveredIndividualGrp/CoveredIndividualName/PersonMiddleNm[lineNum:129687

When I go to look at this line number, I can't find anything wrong with this dependent's name. I even tried deleting him from the form and re-adding, but it was rejected again with the same error message.
