Media Object Image is not showing up when viewing.

I have created new media object Structure and we are storing the data on the server and the Media Object file F00165 has the correct keys, object type and file name of the server location of the file but it does not display the image only shows the below screen.

I can view the image on the server that is stored. Any inputs, what will be wrong.

We are on Tools 9.2.6.5.
