Assistance with Calculation table setup

We are adding a new benefit that the premiums are based on age bands, but will not increase as age progresses. Does anyone have an example of setup that would be similar to this? The calculation table would need to look at employee age at time of enrollment and apply the proper rate, but never change as the employee increases in age.

We are currently on JDE 9.2.
