Approvals by Email

Is there anyway to determine WHY approvals by email fail to update the record? If user is simply clicking "Send" in the generated email (after clicking one of the actions in the email) and not modifying the email in the To line or the data in the subject line; why else would the approval fail?

We are currently a self-hosted client. And we are seeing some transactions work but others fail.
