SPARC T7-1 powers off with "fault.chassis.voltage.failing"

Happens both under load and when idle. Clearing the fault and power up does not work, the fault returns. Clearing the fault, removing power for a few minutes, then powering up succeeds after several tries, and system stays stable for a few days / weeks.

Started happening after firmware and Solaris (11.3 -> 11.4) update, downgrading firmware to the old version did not help, so reinstalled the newer one again.
