How do I set up local tax for Boone County, KY?

I'm using Oracle EBS Payroll 12.2.10

We have an employee working remotely in a home office in Boone, KY. Boone County has 3 separate local withholding taxes.

Only 2 are being withheld from his pay, but the employer is responsible to withhold all 3 so I believe he is set up incorrectly. There should be 3 different taxes and the taxable wages will be different. We recently outsourced our payroll tax function to ADP and I need to be able to feed these values over to them in the interface.

Any help is appreciated.
