Previously Issues LPNs Appearing on Cycle Counts

We are a WMS enabled Org and we (1) part number across (16) different LPNs that have been issued from on-hand inventory months ago, but are appearing on our cycle count today. When we reject them, they still resurface a month or two later on a cycle count again. I attached a snip with some additional details around our issue. Has anyone experienced this before and know what the path is to resolve this issue?
