Replenishment MO creating multiple lines for same item

After running Min/Max, we're seeing that the created MO's rather than have a single line for 25 pieces, are creating 25 lines of 1 piece each. It's not all the time but it's more frequently happening. We're excluding cases for Fied Multiple Lot, and such, that could limit the requested number of items per line.
