PeopleSoft Questionnaire Tables deleted from - Telework tile

We are on PUM 42, 9.2 PeopleSoft, PeopleTools, 8.59.14. Occasionally we have submitted Telework   requests where the data is somehow deleted from the Questionnaire tables.


I do not know what causes this or how recreate this but I was wondering if anyone else has had this happen?

For instance nothing is in PS_EOQF_QUESTIONRE or PS_EOQF_QSTNRCNTXT for the person who submitted the Telework  request.


I am not logging a case yet since I do not know how to recreate it.
