Why would meta data changes not get into the database but provide no error report?

Version 11.2.8.0.0.05

When I make changes in meta data (i.e. add an item) I try to load it, it will not go through. The scan comes back clean, there is no "error" code or report or anything in the log that is different than in the past changes and the load works in our Test instance (and log looks the exact same as production). The only difference is there is no successful load and a red X is next to the Log button.

Is there a way to troubleshoot this?
