ADOP FAILED: Failed to execute SQL statement ad_zd_adop.INSERT_ADD_NODE_RECORDS;

When applying a patch using ADOP utility it fails with error" Failed to execute SQL statement ad_zd_adop.INSERT_ADD_NODE_RECORDS;". I'm using Ebs 12.2.4 with  12.1.0.2.0 Dabatase.

Please help solve this error.
