Workflow Notifications shows SSHR request approved but data not on SIT Form

R12.1.3

Hello Folks,

 

As titled workflow notification shows that employee submit the self-service request and approved by approval

But on related special information form this record not there!

Also checked on Table WF_ NOTIFICATIONS and show that Status = Closed

 Any advice how to know what happened to this approved self service request?
