My 2-node cluster, VIP(INTERMEDIAE FAILOVER) and DB status ( OFFLINE)

My oracle product is 11.2.0.4.0 Enterprise Edition,

When I was checking the 2-node RAC cluster status by using crsctl stat res -t, I was getting the VIP resource status as INTERMEDIATE FAILOVER and instance DB1 state is showing OFFLINE. could you please help me to resolve the error ASAP.
