Pay Advice duplicate number in same payroll

We had a payroll that produced duplicate advice numbers on different employees as if the starting number on the Form table reset itself. The Pay Confirm process did abend and we had to fix an employee and rerun the Pay Confirm. This is not the first time confirm has abended and we reran it without any issue. Any thoughts?
