On EBS 12.2.6, Total Comp Wizard error "progression point has already been defined"

On EBS 12.2.6, HR staff is running the Total Compensation Wizard for grade step rate changes, and when they get to step 7 at the end, they select to Activate Grade Ladder and submit, then it returns this error:

"This progression point has already been defined for this scale rate. Please enter a different progression point or scale rate."

They can run the process in our DEV environment (cloned in March) without issue. I am planning on bouncing our Apps and DB tonight, and staff already tried to clear browser cache. Does anyone know what this error means?
