Fluid Application for Graduation "add name" issue

When attempting to add a new name during the Application for Graduation, we get the following error:

I've dug through the delivered code and it is not saving any of the values that were entered on the form. The name type setup has been completed and seems to be working fine but this error message is displayed because FIRST NAME and LAST NAME are null despite having been entered on the form. Is this working correctly for anyone else?
