P0150 Duplicate Key not allowed.

JDE E1. 9.1

.Raphael will approve as level one and level 2 but I get this error about duplication when adding Raphael for both levels. If we keep Raphael at $5000, anything below 5K is not being approved and procurement cannot see any approvals. Please see the matrix screenshot
