Why are Ohio City taxes being withheld if Ohio Municipal Income Tax Address show no taxes?

Why are Ohio City taxes being withheld when the Ohio Municipal Income Tax Address lookup tool says that there should NOT have any city taxes withheld? We have two employees living in Ohio, 1 employee lives in Delaware County, City of Westerville and 2 employee lives in Warren County, City of Maineville. When we checked with both the Ohio Municipal Income Tax Address Lookup tool and then asked the County Auditor's Office. Both sources indicated that both of these employees' addresses should not be withholding city taxes. What within Oracle is causing the City Taxes
