Running into issues on auto creation of IGT Order Modification based on an approved Requisition

We use iProcurement and Orders are created from requisition approval workflow. We modified PO_IGTORD_CUSTOM to autocreate IGT Orders when a requisition is approved. But unable to created modifications using the same custom hooks. The IGT Order is getting created as a new order and NOT as an IGT Order Modification. Has anyone had luck in creating automatic IGT Order Modifications??
