Wellness Activity - Self-Service Attestation Options

Are you using PeopleSoft HCM to allow employees the capability to self-attest certain wellness related activities?

We are considering using something like the delivered pages in the Vaccination sections of the Health & Safety modules to enable employees the option of self-attesting (uploading related documentation) completion of certain Wellness activities like 5K races, regular exercise at gyms, etc.

Is anyone using something already without much customization?
