why did PV_AUTO_SRC create duplicate PO's?

We have PV_AUTO_SRC running on a 15 minute recurrence. Two back to back jobs processed the same requisitions, resulting in duplicate PO's for these requisitions and PO total quantities double the requisitions. This happened once yesterday and not again since. This suggests to me that the earlier PV_AUTO_SRC job instance failed to update the requisitions's Sourcing Status from Ready to Complete, but was successful on the second instance.

I'm confused about how this could have happened. I've reviewed the process monitor and could not find any other PV_AUTO_SRC jobs running at the same time as our scheduled job.
