Need help for an HMR to modify from header

could someone please help in performing the below change using HMR :


FROM header coming from Provider :


From: <tel:0527756896;noa=national;srvattri=national>;tag=fnde4ble

Please note from number would be different on every call.

From header we need is as below 

From: <sip:0527756896@10.X.X.X;user=phone>;tag=fnde4ble.

Here the IP address is the IP we need which is coming in TO header.. and TAG would remain same ...
