SQL query to get the PR approval workflow if there is a Request for information action is performed

Hi All, Can anyone please provide me the query to be run on toad for pending approval for PRs, basically the PRs which are in 'in process' status and there is a 'Request For Information' Action is performed.

 The output should contain, PR no| Who asked for more information| when its asked| What is the Comment| Pending with whom
