Wanting to find out if anyone has experienced grants assign to roles spontaneously disappear.

I wanted to find out if anyone has experienced grants assigned to a role spontaneously disappearing. I have seen this problem in 12.1 EE and now in 19 SE(RDS Oracle). The grants are still set in the role but access fails. When I explicitly assign the grants directly to the user/schema, they do not disappear. This is not immediate and takes a few months till i get a random call from a customer that permissions have changed. Also the databases never get upgraded automatically, the 12.1 database have never been upgraded(application vendor requirement) and the 19 SE are databases are
