Quote Form is not diaplaying fnd_attached_documents at header level

Version 12.2.5

Quote Creation Form is not displaying Attachments created via fnd_attached_documents_pkg.insert_row. Eventhough fnd_documents and fnd_attached_documents are having data via call to API fnd_attached_documents_pkg.insert_row, Attachment is not visible from fron-end EBS screen. Please help. Attaching my code for your reference. Please help if I am missing something in the code or if it has to be attached in a different way, then I need some pointers.
