How to implement Fusion Tax for US by uploading the rates from a tax rates provider.

We are trying to implement Fusion Tax for US by uploading the rates from a tax rates provider. Note that tax rates will be loaded in Oracle fusion tax and we will setup the rules for tax to be auto calculated. 

I am looking for a file template that we need to import into UCM folder before running the "Tax Configuration Content Upload Program" ESS job. I am referencing to the below content where it says "Data File includes", I am looking for this data file template, that we can prepare and place in UCM folder / tax / import and run the below job.
