OEM Cloud Control Agent Status Fails with Message Connection refused for Managed servers only

OEM Grid Control 13.5

Having issue adding WebLogic middleware components to OEM.

The Admin is the only successfully added while the domains managed servers are showing down although are up and running.

"Failed to connect to the WebLogic Server: cannot communicate with Target: java.io.IOException: t3://hostname:8023: Destination 0:0:0:0:0:0:0:1, 

8023 unreachable; nested exception is: java.net.ConnectException: Connection refused (Connection refused); No available router to destination"
