Hi,

I am looking to prepare a qualification document for my Organization which should be issued by my Quality team before and after the patch deployment. Though I have prepared something already but needed to check if there's any industry standard format or even something which is easy yet effective to fill out before and after Patch deployment in the Oracle EBS environment.
