Description of Problem:

Can we create a template/form for issuing CERTIFICATES to vendors on Withhold Tax paid in format having details like: Withholder name, Address, TIN, Vendor name, Month, Payment date, Withhold tax paid, fines, total payment made, signature, designation.

The format attached for reference.
