Can you help with a SQL query for payroll run results for source & indirect result elements?

We've been asked to write a payroll run results report and I'm encountering difficulties with the SQL logic required to display the results in a single row for the source information element and the indirect earnings element.

The information element contains five input values (shaded in blue) that need to be displayed with the corresponding pay value (Amount).

Any guidance would be much appreciated.
