System Extract file creating Duplicate Rows

We have a system extract process that creates an enrollment file for Blue Shield. It's randomly creating duplicate rows for certain record types for dependents. Has anyone encountered this before? Any help is highly appreciated!

Here's one example (for the same dependent):

HD*021**PPO*PPOX0002*ECH~
DTP*348*D8*20230101~
HD*021**PPO*PPOX0002*ECH~
DTP*348*D8*20230101~
